We may combine information from the Site and Services together and with other information we obtain from our business records. We may use and share information that we aggregate (compile to create statistics that cannot identify a particular individual) or de-identify (strip information of all unique identifiers such that it cannot be linked to a particular individual) at our discretion.

·       Exercise equipment manufacturers.

·       Credit reporting agencies, when you apply for membership using a credit card.

·       Banking verification services & financial institutions. We may use financial institutions to link and verify the bank account information maintained by your financial institutions. This bank account information may include account and routing numbers and related personal information. By providing us with your personal and banking information, you grant us the right, power, and authority to act on your behalf to access, transmit, and receive such information from the relevant financial institutions.

·       From others you specify. If you decide to invite others to the Site or Services, we will collect your and the other person’s names, email addresses, and/or phone numbers to send an email or text message and follow up with the other person. You agree that you will obtain the other person’s consent before giving us their contact information. You also agree that you will not send us the contact information of a minor. We will inform any other person you invite that you gave us their information in the invitation email.

Other businesses in the context of a commercial transaction: We may change our ownership or corporate organization while providing the Services. This is a commercial website intended for use by individuals that are at least 18 years old. Healthtrax Fitness & Wellness and it’s facilities, does not accept members under the age of 13 and does not knowingly collect information from anyone under the age of 13.

INTERNATIONAL TRANSFER AND APPLICABLE LAW

For visitors using the Site or the Services from outside the United States, any data you provide will be transferred to the United States for use by Healthtrax Fitness & Wellness and it’s facilities, and/or its affiliates for any of the purposes described in this Policy. Request Response Procedure and Identity Verification

Only you, or someone legally authorized to act on your behalf, may make a verifiable consumer request related to your personal information. You may also make a verifiable consumer request on behalf of your minor child.

The verifiable consumer request must:

  • Provide sufficient information that allows us to reasonably verify that you are either the person about whom we collected personal information or an authorized representative. We may need to follow up with you to request more information to verify your identity.

  • Describe your request with sufficient detail that allows us to properly understand, evaluate, and respond to it.

We cannot respond to your request or provide you with personal information if we cannot verify your identity or authority to make the request and confirm the personal information relates to you.

Making a verifiable consumer request does not require you to create an account with us.

We will only use personal information provided in a verifiable consumer request to verify the requestor’s identity or authority to make the request.
